Click the radio button next to the option in the field admin to set that as the default. If you want to do it based on which page they came from, you'll have to explore other means such as jQuery.

No, that's not available. You'd have to manually build the address fields based on individual items.

The problem is that your child theme's stylesheet is called too early. You need to make sure it comes after the VFB CSS files in order for your changes to override.
