I love your plugin and have it running on several sites.
I have a client who wants a "request a quote" form on several pages, each page featuring a specific service. I want to use the same form on all the pages, but I want the form results to indicate WHICH PAGE it came from.
In other words, I want to know which page the visitor was on when s/he filled out the form.
Is that possible to do?
