Mark on "[Plugin: Visual Form Builder] Can't create my first form"

Hi,

I've just installed the plugin, but when I click to get started, fill in the fields and click on the 'Create form' button, the screen just goes back to the first screen. I've tried this as admin and an editor. I've tried deactivating and reactivating the plugin.

Anyone come across this before?
