The word "Fieldset" is just the name of that field. You can remove the name there by clearing it out from the VFB admin.
As far as the "brackets", it looks like you have the shortcode wrapped in a <code> tag. Go into the Code view and make sure the VFB shortcode doesn't have any extra tags around it.
