fields missing mystery

I built a form with probably 25 fields on it and saved it. When came back to the site and I try to edit the form it is missing most of the fields. I dont see them anywhere, however if I go to export the form, all the fields are there that were saved.

If I put it on a draft page only one field is there

I am confused, can anyone shed light on this puzzle?
