MarketingSE on "[Plugin: Visual Form Builder] using for a request form"

I would like for our Literature request form to have a way to list out the items available to order (as they currently are) but instead of the checkbox, have a field next to each listed item where they can type the qty they wish to order (a numbers or text field). Is that doable with this form?
