windstyles on "[Plugin: Visual Form Builder] 4 columns for checkboxes?"

I am in need of a field that allows for our scholarship applicants to note what activities they did throughout high school and what years they did those activities. For example:

The headers would be:
Activity Name 9th 10th 11th 12th

Obviously, we cannot get this to work as is, because it only allows for 3 columns.

And then we'd need 3 rows for them to enter the above info. It is possible that some activities were done in more than one year of school, so we'd like them to be able to select multiple grades that go with that activity. (Hope this makes sense).

So, we either need a way to put the check boxes into 4 columns, or a dropdown that accepts multiple answers... Thinking having more than 3 columns in the form is the best/easiest way.

Can anyone provide info on how to allow for more than the standard 3 columns, please?
