dannyogolo on "[Plugin: Visual Form Builder] How to place form in a Page or Widget"

I Just installed this wonderful Plugin, looks very promising.
But after building a form, I could not figur out how to place it on the page I want it to appear.

Also I intend to build another instance of the form with fewer fields for my widget. How do I do this ?
